Acts as case manager and team lead for patients requiring home health therapy services, when nursing is either not involved in the case or physical therapy is the primary patient need. Assesses the patient and collaborates with the interdisciplinary team and primary care provider to develop the patient-centered plan of care. Provides physical therapy treatment to the patient/family/caregiver as ordered. Completes accurate clinical documentation to effectively support service provided within specified timeframes Provides physical therapy to pediatric, adolescent, adult and geriatric patients based on principles of growth and development and life stages. Provides instruction to the patient/caregiver and revises the treatment plan as appropriate. Maintains communication regarding patients with the case manager and other disciplines via care conferences or other appropriate methods. Completes regulatory required assessments within specified timeframes. Completes medication reconciliation at specified timeframes. Qualifications: 1-2 years outpatient rehabilitation setting. At least two (2) years of physical therapy experience highly preferred. One (1) year experience in home health or other health setting with geriatric emphasis. Current license in Physical Therapy or ability to obtain prior to hire. American Heart Association HealthCare Provider BLS preferred upon hire, and required within 90 days of hire. Current Driver’s license Excellent customer service skills Ability to work without direct supervision. Provides care appropriate to the age of patients served on the job. Knowledge of physical therapy assessment and treatment methods Knowledge of physical therapy modalities and equipment Ability to maintain effective relationships with other health care professionals Must be able to develop skills in use of technology used in home care setting.
