Qualifications: * 1-2 years outpatient rehabilitation setting. * At least two (2) years of physical therapy experience highly preferred. * One (1) year experience in home health or other health setting with geriatric emphasis. * Current license in Physical Therapy or ability to obtain prior to hire. * American Heart Association HealthCare Provider BLS preferred upon hire, and required within 90 days of hire. * Current Driver’s license * Excellent customer service skills * Ability to work without direct supervision. * Provides care appropriate to the age of patients served on the job. * Knowledge of physical therapy assessment and treatment methods * Knowledge of physical therapy modalities and equipment * Ability to maintain effective relationships with other health care professionals * Must be able to develop skills in use of technology used in home care setting. * Demonstrate the highest level of professionalism when interacting with patients, families, physicians, and hospital staff members.
