Travel Licensed Master Social Worker (LMSW) - $2,530 per week in Windsor, VT - Job Title: Social Worker Profession: Social Worker (LMSW) Specialty: Case Management Duration: 13 weeks Shift: 8:00 AM - 4:30 PM Hours per Shift: 8 hours Experience: Minimum 3 years clinical experience required; recent acute care hospital experience required License: MSW Degree required; Vermont MSW license required Certifications: Active American Heart Association BLS required Must-Have: - Strong communication skills - Collaborative team player - Creativity and innovation - Flexibility and receptiveness to change - Commitment to professional growth - Computer skills Description: Manage and coordinate interdisciplinary care of defined populations through the care continuum from wellness through end of life. Identify high-risk patients requiring ongoing coordination of care. Perform a comprehensive patient and family assessment. Develop a comprehensive treatment plan addressing clinical components spanning psychosocial issues. Implement a plan of care to provide ongoing support and coordination for patients and families with complex system needs. Manage benefits and negotiate continuing care services for enrollees in various health insurance plans. Utilize innovative strategies to advocate for patient needs and negotiate complex systems. Monitor patient transitions across and within care settings, including home, clinic, skilled nursing facility, rehabilitation, and hospital. Share assessment and care plan data with patient and family consent as the patient moves through different care settings. Identify gaps in the care continuum and work with community and provider networks to expand access to needed services. Participate in the development of clinical disease management strategies and identify appropriate measures for evaluating outcomes. Contribute to the development, maintenance, and coordination of an interdisciplinary care delivery system specific to individualized patient needs. Promote effective resource utilization. Collect and evaluate clinical and financial data outcomes, including patient satisfaction and resource utilization. Perform other duties as required or assigned.
