The Physical Therapist will evaluate, develop, and implement individualized treatment plans to help patients improve mobility, reduce pain, and restore functional independence. Key Responsibilities Evaluate patients and develop individualized treatment plans Implement therapeutic exercises and rehabilitation programs Assess patient progress and modify treatment plans as needed Educate patients and families on treatment goals and home exercise programs Maintain accurate and timely clinical documentation Collaborate with physicians and interdisciplinary healthcare teams Ensure compliance with facility policies and professional standards Promote patient safety and quality care outcomes Assignment Details Start Date: 07/06/2026 End Date: 10/03/2026 Shift: Day Shift Shift Time: 08:00 AM 04:30 PM Schedule: Monday Friday Contract Length: 13 Weeks Guaranteed Hours: 35 per week Requirements Active Physical Therapist (PT) license Previous rehabilitation or skilled nursing experience preferred Strong clinical assessment and treatment planning skills Excellent communication and patient care abilities Ability to work independently and collaboratively within a multidisciplinary team Valid state/federal photo ID required onsite at all times Compliance with onboarding and facility requirements Must maintain all required credentials and certifications
