About the Position
Job Title: Physical Therapist Job Specialty: Home Health Physical Therapy Job Duration: 13 weeks Shift: Day Shift, Monday - Friday, 8 AM - 5 PM Guaranteed Hours: 40 hours per week Experience: Minimum of 1 year License: ND Physical Therapy License Certifications: BLS Must-Have: - Current Driver's License & Auto Insurance - PT-related skills & previous home health experience Job Description: - Provide home health physical therapy services to clients in a post-acute care setting, including those recovering from surgeries and falls. - Cover a 60-mile radius area, treating 5-7 patients per day. - Utilize EMR: HomeCare/HomeBase for documentation. - Conduct services independently, with no clinical involvement or manager on shift. - Orientation will range from 3-5 shifts. - Adhere to dress code: CSH-colored scrubs or business casual. - Manage care-setting: Home Health and Hospice patients. Note: No weekend or holiday shifts are required. Ability to self-schedule and block scheduling are not applicable.
