StartDate: 3/11/2024 Available Shifts: 8 D Pay Rate: $1340.00 - $1544.00

OT-outpatient needed ASAP Fingerprinting must be submitted and cleared prior to start. ** REQUIRED: Bachelor’s in Occupational Therapy from a school accredited by the American Occupational Therapy Association's Accreditation Council for Occupational Therapy Education (ACOTE). Must possess a valid, current, full and unrestricted license from the American Occupational Therapy Association and Arizona Department of Health Licensure or in any state within the United States, District of Columbia, or the Commonwealth of Puerto Rico, or eligible for licensure in Occupational Therapy in State of Arizona. Must have and maintain a current Basic Life Support (BLS) certification by the American Heart Association. Applicant must meet requirements to be credentialed to serve on the Medical Staff. All related information required for credentialing must be provided. ORIENTATION: Every two (2) weeks only. COVID EXEMPTIONS: We do not accept religious exemptions. If a candidate submits for a medical exemption, we will have to get approval from our medical review board to proceed with onboarding. HOUSING APPLICATION: must be submitted 2 weeks prior to start date - form uploaded in Medefis
