Occupational Therapist – Home Health

Contract | Days (5x8 Hours, 8:00–17:00) | Field-Based / Home Health Visits

Key Responsibilities

• Conduct skilled home health visits, completing a minimum of 6–8 patient visits per day
• Perform occupational therapy evaluations and develop individualized treatment plans for patients with conditions including amputations, CVA, TBI, spinal cord injury, Parkinson's disease, multiple sclerosis, and upper/lower extremity injuries
• Provide training in Activities of Daily Living (ADLs), home exercise programs, and safety and injury prevention strategies
• Complete OASIS assessments and maintain accurate billing and clinical documentation using facility-designated systems
• Educate patients and families on therapeutic techniques, adaptive equipment, and ongoing care plans
• Self-schedule visits and hours based on the needs of the home care office

Required Qualifications

• Active Florida OT license or valid compact license required at time of submission
• Minimum of 2 years of occupational therapy experience, with home health experience required
• Current BLS certification
• Valid driver's license and reliable personal vehicle for conducting home visits
• Proficiency in ADLs, orthopedics, prosthetics/orthotics, evaluation tools, and clinical documentation/billing
• Must meet facility health compliance requirements including COVID-19 vaccination or approved exemption, and flu shot or declination with mandatory mask wearing

Preferred Experience / Skills

• OASIS training or prior OASIS completion experience
• Experience with Cerner electronic health records
• Familiarity with home health scheduling and timesheet documentation systems
