Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting
• Evaluate patients and develop individualized treatment plans to address functional deficits
• Deliver therapeutic interventions aimed at improving mobility, strength, and overall function
• Document patient progress and maintain accurate clinical records in accordance with regulatory standards
• Collaborate with interdisciplinary care team members to coordinate patient care

Required Qualifications

• Active Physical Therapist (PT) license in the state of Minnesota or eligibility to obtain
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification
• Ability to work a Sunday through Saturday work week on a day shift schedule

Preferred Experience / Skills

• Prior experience in a home health setting
• Strong communication and patient education skills
