About the Position

Job Title: Occupational Therapist - Home Health Profession: Occupational Therapist Specialty: Home Health Duration: 13 weeks Shift: Day Shift, M-F Hours per Shift: 8 hours Experience: Home health experience is preferable but not required License: Occupational Therapy licensure in all applicable states of practice required Certifications: Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from an approved vendor is required. Must-Have: Valid Driver's License and must be at least 21 years of age or older Description: Our home health and hospice agencies serve the entire community and offer personalized in-home care. The role involves planning and conducting segments of treatment programs designed to restore and improve physical, social, and mental functions. You will serve as a co-leader in an interdisciplinary approach to patient groups. Essential functions include completing evaluations to ensure quality standards and addressing assessment areas that require attention. You will utilize appropriate clinical techniques and tools to establish individualized treatment plans for patients. Providing therapy interventions using appropriate treatment protocols and techniques is key to the role. You will also establish individualized care and discharge plans, ensuring communication with appropriate parties. Completing comprehensive and accurate clinical documentation within established deadlines is required. Supervision of assigned assistants will be part of your responsibilities.
