About the Position

Job Title: Physical Therapist Home Health Profession: Physical Therapist Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8:00 AM - 4:30 PM Experience: 1 year License: Required Certifications: Required Must-Have: - Minimum of 1 year of certified home care experience. - Minimum of 1 year of case management experience. - Experience must be specifically within a certified home health agency. - Ability to travel to cases as assigned. - Weekend rotations required once every 6 weeks. Description: As a Home Health Therapist, you will be responsible for assessing patient conditions. You will implement care plans tailored to the individual needs of your patients. Ongoing education and support will be provided to patients and their families. Patient care must be delivered in accordance with organizational policies and standards. You will be required to perform admission processes and case management responsibilities. Documentation and communications must be thorough and timely. The position involves working with an interdisciplinary team to enhance patient outcomes.
