Join our team in Stockton, CA, as a Home Health Physical Therapist for an 8•week contract, with potential extension, working 40 hours per week during day shifts.

**Responsibilities:**

• Deliver physical therapy in home health settings.
• Document care using Home Care Home Base.
• Conduct initial and ongoing therapy sessions.

**Qualifications:**

• CA Physical Therapist license.
• 1•2 years of home health or hospice experience.
• Proficient in Home Care Home Base.
• BLS certification.
