Occupational Therapist (OT) – Home Health

Key Responsibilities

• Conduct occupational therapy evaluations and revisits for home health patients across the assigned service area
• Complete an average of four patient visits per day, including a combination of initial evaluations and follow-up revisits
• Provide skilled occupational therapy services to patients in their homes within the northeastern Sacramento, Folsom, and Woodland/Yolo County service areas
• Maintain accurate and timely clinical documentation in accordance with home health standards
• Travel independently to patient homes throughout the designated coverage area using a personal vehicle

Required Qualifications

• Active California Occupational Therapist (OT) license
• Minimum of 2 years of experience in a healthcare setting
• Current Basic Life Support (BLS) certification
• Active National Provider Identifier (NPI) number
• Valid driver's license and proof of current automobile insurance

Preferred Experience / Skills

• Prior home health experience strongly preferred
• Certified Occupational Therapy Assistant (COTA) with home health experience may be considered in lieu of OT applicants
