Key Responsibilities

• Assess patient conditions and develop and implement individualized care plans in a home health setting
• Provide ongoing education and support to patients and their families throughout the care process
• Perform admission processes in accordance with organizational policies and standards of practice
• Carry out case management responsibilities, including interdisciplinary communications and coordination of care
• Maintain thorough and timely electronic documentation in compliance with organizational standards
• Travel to assigned patient cases and participate in weekend rotations on a scheduled basis

Required Qualifications

• Active Physical Therapist license in the state of Massachusetts
• Minimum of 1 year of experience in a Certified Home Health Agency (CHHA)
• Minimum of 1 year of Case Management experience within a certified home health setting
• Experience in hospice, private duty, or standalone case management roles does not satisfy the certified home health requirement
• Ability to travel to patient cases as assigned

Preferred Experience / Skills

• Familiarity with electronic medical records (EMR) systems used in home health environments
• Strong interdisciplinary communication and care coordination skills
