Key Responsibilities

• Evaluate and treat patients in a Skilled Nursing Facility (SNF) setting
• Develop and implement individualized physical therapy treatment plans
• Monitor and document patient progress toward established therapy goals
• Collaborate with interdisciplinary care team to ensure comprehensive patient care
• Educate patients and families on therapeutic exercises and home programs

Required Qualifications

• Active Physical Therapist (PT) license in the state of Kentucky or eligibility for licensure
• Graduate of an accredited Physical Therapy program
• Current CPR/BLS certification
• Experience working in a SNF or similar clinical setting

Preferred Experience / Skills

• Prior travel or contract therapy experience
• Familiarity with skilled nursing documentation and Medicare guidelines
