Contract (13 Weeks) | Full-Time | Days | Monday – Friday | Home Health

Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting
• Evaluate patients and develop individualized treatment plans based on clinical findings
• Implement therapeutic interventions to improve patient mobility, strength, and functional independence
• Monitor and document patient progress, adjusting treatment plans as clinically appropriate
• Coordinate care with the interdisciplinary home health team to ensure continuity of patient care

Required Qualifications

• Active Physical Therapist (PT) license in the state of Texas
• Graduate of an accredited Physical Therapy program
• Experience providing physical therapy in a home health setting
• Valid driver's license and reliable transportation for patient home visits

Preferred Experience / Skills

• Prior experience in a home health or community-based therapy environment
• Familiarity with home health documentation and compliance requirements
