About the Position

Job Title: Physical Therapist - Home Health Profession: Therapy/Rehabilitation Specialty: Physical Therapy Duration: 13 Weeks Shift: Day Hours per Shift: 8 Hours (08:00 - 17:00) Experience: Minimum 1 year of adult home health experience License: Must have a valid license Certifications: CPR, BLS Must-Have: Recent home health experience Description: Provide home visits for physical therapy patients. Attend periodic office meetings. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Average number of patients per day is 5 to 6. Contractor will be assigned between 5 to 10 patients per day. Each visit and activity, including documentation and travel, has a different point value. The contractor should reach a productivity expectation of 30 points per week. Types of cases include start of care and follow-ups. Must be willing to travel up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate. Travel is compensated from patient home to patient home. The commute to and from the first and last patient is not billable. No guarantees of hours are provided. If patient census is low, contractors can discuss floating to a nearby location to pick up additional hours. Must be available for weekend rotation as required. No on-call duties are expected. Documentation must be completed within 24 hours of the appointment. Delayed and incomplete documentation may impact patient care and billing. All clinicians must accept every patient on their schedule regardless of distance. Must be proficient in specific EMR systems. Local candidates will be accepted. Must acknowledge the responsibility of driving to patients' homes daily.
