Job Title

Physical Therapist – Home Health

Location

Portland, OR (East Portland and Clark County, WA and Surrounding Areas)

Schedule / Job Type

Contract | Days | Full-Time | 5x8-Hour Shifts (8:00 AM – 4:30 PM) | Onsite / Field-Based

Key Responsibilities

• Provide skilled physical therapy services to home health patients in a 1:1 care setting, visiting up to 6 patients per day
• Complete clinical documentation using EPIC Remote Client for Home Health and Rover mobile app
• Conduct and complete OASIS assessments accurately and in a timely manner
• Manage a daily patient schedule across a service area, with occasional drive times up to 60–90 minutes per day
• Maintain 30 productivity points per standard shift in accordance with home health guidelines

Required Qualifications

• Active Physical Therapist licensure in both Oregon (OR) and Washington (WA) required
• Minimum 1 year of skilled home health experience required
• OASIS experience required
• Current Basic Cardiac Life Support (BCLS) certification required
• Must have reliable personal transportation

Preferred Experience / Skills

• Experience with EPIC electronic health record system in a home health setting
• Familiarity with Rover mobile documentation application
• Strong time management and self-directed scheduling skills in a field-based environment
