Contract | Full-Time | Onsite | Day Shift | Monday–Friday | 40 hours per week | 8-hour or 10-hour shifts available

Key Responsibilities

• Provide direct physical therapy services to patients in an outpatient clinical setting
• Evaluate, assess, and develop individualized treatment plans based on patient needs
• Deliver therapeutic interventions and monitor patient progress toward goals
• Complete required clinical documentation accurately and in a timely manner
• Collaborate with interdisciplinary team members to support patient care outcomes

Required Qualifications

• Active Physical Therapist license in the state of Arizona (permanent license required)
• Prior experience in an outpatient physical therapy setting
• Ability to meet facility-specific credentialing and documentation requirements prior to assignment start
• Must have been separated from any prior employment at this facility for at least one year to be eligible

Preferred Experience / Skills

• Experience working in a high-volume outpatient rehabilitation environment
• Familiarity with contract assignment workflows and travel therapy expectations
