The team will work with the primary care team to accomplish various tasks. Social support navigation for social determinants of health (SDOH) will be provided. This includes addressing issues such as food insecurity and housing insecurity. Compile and maintain a resource list for SDOH resources. The resource list includes eligibility criteria, referral process, and contact information. Collaboration with primary care nurses and providers will be essential. In-person or remote social needs screening/assessment will be conducted. This screening is for primary care patients referred by nurses or providers. Coordinate or make aware of social services resources such as housing, clothing, food, and mental health services. Collaboration with other social workers to identify patient and community resources will occur. Conduct case management activities will be part of the role. Work with hospitals on discharge planning, follow-up, and education. Assistance will be provided in obtaining patient records from hospitals. The social worker will assist in securing needed medical equipment through community partners. Follow-up on care plans is a key responsibility. Identifying patients lost to follow-up or overdue for care is necessary. Assist such patients in returning to care. The team may assist with specialty referral navigation. Scheduling, coordinating, and tracking referrals is expected. Help will be provided in obtaining patient records from specialists and imaging centers. Compile and maintain a resource list for specialty referrals, including eligibility criteria and contact information. Patients will be assisted in locating and accessing low-cost prescription options. This includes patient assistance programs and discount retailers. The social worker may assist with patient assistance program applications. Serve as a liaison with drug companies on behalf of patients. Assist patients with applications for programs related to prescription access. May help with other regional primary care-based initiatives with a social work component. Documentation in patient records and updating consults will be required. Tags for providers or clinical staff will be made as necessary. Provide patient education or find appropriate educational resources. Expectations may include completing onboarding and orientation. Participation in regional office and primary care clinical meetings may be requested. Attendance at provider meetings may be necessary. Engagement in community meetings to build relationships with social service agencies is expected. Identifying barriers to care experienced by patients and seeking solutions is a crucial task. Tools and equipment used include a personal computer, telephone, fax machine, printer, scanner, copy machine, calculator, personal vehicle, and other office-related equipment as required.
