About the Position

Job Title: Physical Therapist Profession: Physical Therapist Specialty: Physical Therapist Duration: 8 weeks Shift: Day Shift Hours per Shift: 40 hours per week (4x10s) Experience: Minimum of two years’ Physical Therapy experience preferred License: Physical Therapy licensure in state of practice required Certifications: Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification required Must-Have: Recent EPIC experience Description: Plans and conducts segments of treatment programs designed to restore and improve physical, social, and mental functions. Meets department objectives while serving as a co-leader in an interdisciplinary approach to patient groups. Completes evaluations to ensure quality standards and addresses assessment areas that require attention. Uses appropriate clinical techniques and tools to establish individualized treatment plans for patients. Provides therapy interventions using appropriate treatment protocols and techniques. Establishes individualized care and discharge plans, ensuring plans are communicated to appropriate parties. Completes comprehensive and accurate clinical documentation within established deadlines. Supervises assigned assistants. Performs other job-related duties as assigned. Is committed to the safety and well-being of associates and patients. Requires all associates to receive all required vaccinations as a condition of employment, with medical and religious exemptions applicable. Participates in E-Verify.
