Key Responsibilities

• Deliver physical therapy treatments to residents under the supervision of a licensed Physical Therapist
• Assist in implementing individualized care plans and documenting patient progress
• Support residents in achieving functional goals related to mobility, strength, and daily living activities
• Communicate effectively with the interdisciplinary care team regarding patient status and progress

Required Qualifications

• Active Physical Therapy Assistant (PTA) license in the state of Wisconsin
• Graduate of an accredited PTA program
• Current CPR/BLS certification
• Experience working in a skilled nursing facility or similar long-term care setting

Preferred Experience / Skills

• Prior experience in a SNF or post-acute rehabilitation environment
• Familiarity with Medicare documentation and compliance requirements
