About the Position
Job Title: Clinical Pharmacist
Position Summary:
We are seeking an experienced Clinical Pharmacist to join a hospital-based pharmacy team. This role is responsible for ensuring the safe and effective dispensing of medications and pharmaceutical supplies while collaborating with physicians, nurses, and other healthcare professionals to optimize patient care. The Clinical Pharmacist will provide clinical expertise, support medication management initiatives, and ensure compliance with all applicable regulations and organizational policies.
Key Responsibilities:
- Review, verify, and dispense medications accurately and safely.
- Provide clinical pharmacy services to support optimal patient outcomes.
- Evaluate medication therapies for appropriateness, effectiveness, and safety.
- Collaborate with physicians, nurses, and interdisciplinary care teams on medication-related decisions.
- Monitor for drug interactions, adverse reactions, and therapeutic effectiveness.
- Educate patients and healthcare staff regarding medications and treatment plans.
- Maintain accurate documentation and ensure compliance with pharmacy regulations, accreditation standards, and organizational policies.
- Participate in quality improvement initiatives and medication safety programs.
- Manage pharmaceutical inventory and ensure proper storage and handling of medications.
Requirements
Qualifications:
- Current Pharmacist license in the applicable state of practice.
- Minimum of 2–3 years of recent clinical pharmacy experience required.
- Previous hospital pharmacy experience required.
- Strong knowledge of clinical pharmacology, medication safety, and evidence-based practice.
- Excellent communication, critical thinking, and problem-solving skills.
- Ability to work effectively in a fast-paced, interdisciplinary healthcare environment.
Schedule & Compensation:
- Full-time
- 40 hours per week
- Competitive hourly compensation
Interactions:
This position regularly collaborates with patients, families, physicians, nursing staff, pharmacy personnel, leadership, and other members of the interdisciplinary healthcare team to deliver high-quality patient care.
