Schedule / Job Type

Contract | Full-Time | Days | Onsite | 5 x 8-hour shifts per week, including rotating weekends and holiday rotation

Key Responsibilities

• Perform Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and daily patient visits in a home health setting
• Manage a caseload of approximately 6.5 points per day within the home health territory
• Provide physical therapy services to patients with diagnoses including CHF, orthopedic conditions, and pneumonia
• Document patient care accurately using Home Care Home Base (HCHB) software
• Participate in rotating weekend shifts (8:00 AM – 5:00 PM) and holiday rotation as scheduled
• Collaborate with clinical managers and educators, with 24/7 support available for emergencies and standard business hours support for general needs

Required Qualifications

• Active Physical Therapist (PT) license in the state of Kentucky
• Current BLS certification
• Prior home health experience required
• Proficiency with Home Care Home Base (HCHB) software required

Preferred Experience / Skills

• Experience treating patients with CHF, orthopedic conditions, or pneumonia in a home health setting
• Experienced clinicians preferred; new graduates are discouraged from applying
