Seven Healthcare are seeking an experienced Physical Therapist for a travel assignment in Lenoir, NC. This role is focused on providing expert physical therapy services in the home health care environment, specifically targeting the needs and recovery goals of patients within assigned counties. Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment based on physician orders/referrals
• Complete comprehensive evaluations using appropriate tools and procedures
• Review patient records for current diagnosis and past medical history
• Establish measurable goals and develop treatment plans for patient independence
• Regularly reassess treatment plans and patient progress, modifying as necessary
• Communicate patient needs and progress with treatment team and family members
• Consult with medical providers on patient evaluation, progress, and discharge
• Recommend and assist in securing adaptive equipment to improve patient function
• Maintain documentation in compliance with regulations and company policy
• Supervise Physical Therapist Assistants and Physical Therapy Aides
• Attend and contribute to staff development programs and obtain required education

Requirements of the Physical Therapist

• Valid PT license in North Carolina (NC)
• Experience in home health setting with HCHB experience
• Knowledge of Oasis
