Key Responsibilities

• Deliver physical therapy treatments to patients in a skilled nursing facility (SNF) setting under the supervision of a licensed Physical Therapist
• Assist in implementing individualized treatment plans to improve patient mobility, strength, and functional independence
• Document patient progress accurately and in accordance with facility and regulatory standards
• Communicate effectively with the supervising Physical Therapist and interdisciplinary care team regarding patient status and goals

Required Qualifications

• Active Physical Therapy Assistant (PTA) license in the state of Arizona or eligibility for licensure
• Graduate of an accredited Physical Therapist Assistant program
• Current CPR/BLS certification
• Experience working in a skilled nursing facility (SNF) or similar clinical setting

Preferred Experience / Skills

• Prior experience in a SNF or long-term care environment
• Familiarity with Medicare documentation and compliance requirements
