Key Responsibilities

• Provide physical therapy assistant services to patients in a home health setting across a multi-county territory
• Meet a minimum productivity expectation of 30 points per week
• Complete all clinical documentation by 10:00 AM the following day
• Deliver patient care across assigned service areas within the designated territory

Required Qualifications

• Active Physical Therapist Assistant (PTA) license
• Minimum of one year of recent home health PTA experience
• Proficiency with HCHB (Homecare Homebase) documentation software
• Ability to travel throughout a multi-county service territory

Preferred Experience / Skills

• Familiarity with home health regulatory and documentation requirements
• Strong time management skills with the ability to manage a high-volume caseload independently
