Seven Healthcare are seeking a Licensed Physical Therapist specializing in home health to join our team in Washington County, MD. This contract position involves providing comprehensive physical therapy evaluations and treatments to patients based on physician referrals in a home visit setting, with no requirement for holiday, weekend, or on-call work. Ideal candidates will have experience with Home Care Home Base and the ability to oversee Physical Therapist Assistants (PTAs).

Key Responsibilities of the Licensed Physical Therapist

• Perform physical therapy evaluation and treatment of patients
• Complete comprehensive evaluations using appropriate tools and procedures
• Review patient records for diagnosis, medical history, precautions, and contraindications
• Establish measurable goals and develop treatment plans
• Regularly re-assess treatment plan effectiveness and patient progress
• Communicate patient needs and progress with the treatment team and family members
• Consult with medical providers on patient evaluation, treatment, and discharge
• Recommend and assist in securing adaptive equipment for patient rehabilitation
• Maintain and submit documentation as required by regulations
• Adhere to the Company's Code of Business Conduct and other professional standards
• Supervise Physical Therapist Assistants and Aides
• Participate in staff development programs and maintain clinical practice skills

Requirements of the Licensed Physical Therapist

• Licensed as a Physical Therapist in Maryland
• Experience in home health settings preferred
• Experience with Home Care Home Base preferred
