About the Position
Job Title: Licensed Master of Social Work Profession: Social Worker Specialty: Therapy Duration: 8 Shift: 5x8.0 Hours per Shift: 8 Experience: Minimum 2 years of experience in a hospital setting required. License: Must be licensed in Client. Description: Responsibilities include completing patient assessments that identify needs in discharge planning within the timeframe set forth by departmental policy. Assessments must include biopsychosocial, financial, medical, and physical factors while considering age-specific issues and clinical impressions. Respect the patient's cultural, language, religious, and spiritual values, beliefs, and preferences while accommodating accordingly. Assess the patient's capacity for self-care and the availability and capability of family or friends to provide care at home, if the patient is unable to care for themselves. Protect and promote patient rights. Actively engage patients and/or families to participate in the development and implementation of the care plan. Perform ongoing reassessments of the patient per departmental policy and reflect any changes in the patient's medical or social condition that may require updates to the plan. Provide post-hospital care options to patients and/or families, allowing them the freedom to choose their post-hospital care providers. Coordinate patient care plans by collaborating closely with a multidisciplinary team to develop safe discharge plans based on recommendations. Communicate effectively with the multidisciplinary team and other social work staff to ensure appropriate hand-offs. Demonstrate knowledge and utilization of community-based resources to meet patients' needs while allowing them to continue living independently and maintaining health in the community. Educate and arrange placements in other healthcare facilities for patients unable to return home. Coordinate patient services with payors, the multidisciplinary team, patients, families, and community resources. Advocate on behalf of the patient with referral and payor sources to access needed services. Provide short-term counseling for problem-solving, crisis intervention, bereavement, and adjustment related to medical diagnoses or hospitalization. Perform psychosocial or discharge planning evaluations at the request of patients, families, or physicians regardless of high-risk criteria. Seek guidance from supervisors or peers for cases involving complex psychosocial or discharge planning situations. Demonstrate the ability to set priorities and manage time effectively in a cost-efficient manner. Handle crises or unfavorable situations effectively. Utilize a variety of techniques to assist in resolving patient care-related problems. Exercise time management and prioritization skills in day-to-day practice to achieve result-oriented outcomes. Comply with required standards for medical/legal reporting situations, including child abuse, adult abuse, and sexual abuse. Attend staff meetings, educational in-services, supervision, or trainings to improve performance and the delivery of patient care. Identify and participate in opportunities for practice-based continuous quality improvement activities. Understand and agree that work assignments and time schedules may change based on departmental needs. Perform all other duties as assigned. Understand and agree that work assignments and schedules are adaptable within the scope of services and departmental needs.
