About the Position

Job Title: Physical Therapist Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum of 1 year of adult home health experience License: Must have a valid license Certifications: CPR, BLS Must-Have: Recent home health experience Description: Home visits required for therapy services. Periodic office meetings are part of the responsibilities. Reliable transportation is necessary. Current driver's license and agency-required liability insurance are mandatory. Average number of patients per day is 5-6. Contractor will be assigned between 5-10 patients that equate to roughly 6-8 points. The weekly productivity expectation is 30 points. Each type of visit and activity (documentation, travel, etc.) holds a different point value. Types of cases include start of care and follow-ups. Contractors should be open to traveling up to 75 miles from their home clinic. Mileage will be reimbursed at the IRS standard rate. Compensation starts from patient home to patient home only. The commute to and from the first and last patient is not billable. Local candidates are accepted at the same rate. Official license must be in hand; pending licenses are not accepted. All documentation must be completed within 24 hours of the appointment with the patient. Delayed or incomplete documentation affecting billing and follow-up patient care may result in cancellation. There are no guaranteed hours with this contract. The clinician will be expected to accept every patient on their schedule. Declining patients may also result in cancellation. If there are not enough patients to meet financial needs, there may be an opportunity to float to a nearby clinic. On-call rotations may vary and should be discussed during the interview. Candidates must be proficient in HomeCare HomeBase EMR and Oasis, having used them within the last 6 months.
