About the Position

Job Title: Physical Therapist - Home Health Profession: Therapy/Rehabilitation Specialty: Physical Therapy Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Recent Home Health experience Description: This position is for a Physical Therapist in a home health setting. The therapist will provide in-home care to patients in need of rehabilitation services. The average weekly travel to patient homes will be around 200 miles. The contractor will need to travel to provide care to patients located in various counties. The work schedule is Monday through Friday from 8 AM to 5 PM. There are no weekend rotations or on-call requirements for this position. The therapist will be responsible for seeing an average of 5 to 10 patients per day. Productivity expectations will include achieving 30 points per week, equating to 40 hours of work. All documentation related to patient care must be completed within 24 hours of the appointment. The candidate must have reliable transportation, a current driver’s license, and agency-required liability insurance. It is important that the therapist is comfortable with driving distances and traveling between patients. There are expected periodic office meetings for collaboration and support. Candidates without recent Home Health experience will not be considered. All necessary certifications must be maintained and presented as required. This position does not guarantee set hours weekly but offers flexibility to float to nearby facilities if needed.
