Contract | Full-Time | Onsite | Day Shift (Monday–Friday, 8:00 AM–5:00 PM) | 5x8-Hour Shifts | 40 Guaranteed Hours per Week

Key Responsibilities

• Provide physical therapy services to adult patients in a home health setting across the Montgomery and upper Prince George's County areas
• Conduct approximately 6 patient visits per day within the assigned service area
• Evaluate, plan, and implement individualized physical therapy treatment plans in accordance with physician orders and patient needs
• Document patient care accurately and in a timely manner using electronic health record systems
• Maintain compliance with all applicable clinical and regulatory standards throughout the duration of the assignment

Required Qualifications

• Active Maryland Physical Therapist state license or Compact license — must be active and in hand at time of submission
• Current BLS certification — expiration date must be included at submission
• Minimum of 1 year of physical therapy experience
• Home health experience required
• Must be nicotine-free; a positive nicotine screen will result in cancellation
• Two supervisor references required

Preferred Experience / Skills

• Experience with HCHB (HomeCare HomeBase) electronic documentation system preferred
• Familiarity with adult home health case management and high-volume visit scheduling
