About the Position
Job Title: Occupational Therapist Profession: Occupational Therapist Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours M-F Experience: 1 year of occupational therapy experience required License: Occupational Therapist license required Certifications: None specified Must-Have: Long Term Care or Rehabilitation experience required Description: Effectively screens and evaluates patients with cognitive and/or ADL physical functioning disorders. Develops appropriate plans of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Provides rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders. Administers various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, therapeutic exercises, modalities, and the use of supportive and assistive devices. Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements. Evaluates treatment outcomes for effectiveness and modifies services based on patient evaluation. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Effectively communicates with supervisors and other health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services provided per regulatory and clinical requirements. Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy. Accurately records treatment minutes for services provided per all regulatory and clinical requirements. Participates in required meetings including patient care conferences, utilization review meetings, and rehabilitation meetings. Participates in required trainings, in-services, and conferences. Able to obtain necessary DME or supplies per regulatory and clinical practice requirements. Supervises and supports occupational therapist assistants, rehabilitation aides, and/or students. Adheres to practice acts, code of conduct, and ethics during professional practice as an Occupational Therapist.
