About the Position

Job Title: Physical Therapist Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum of 1 full year of Adult Home Health experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Recent Home Health experience Description: Conduct home visits for physical therapy patients. Participate in periodic office meetings. Must have reliable transportation and a current driver's license. Agency-required liability insurance is necessary. Average number of patients per day is 5-6. Contractor will be assigned between 5-10 patients per day. Documentation and travel duties hold different point values. Contractor will need to cover a territory within a 75-mile radius. Mileage will be reimbursed at the IRS Standard rate. The commute to and from the first and last patient is not billable. Weekend requirements vary by location and profession. Patient types include Start of Care and Follow Ups. Documentation must be completed within 24 hours of the appointment. There are no guaranteed hours associated with this contract. Weekly productivity expectations are set at 30 points. All overtime must be pre-approved. Traveling to multiple patient homes may result in longer drives. Clinician should be comfortable with these driving distances. Declining patients may result in a cancellation of the contract. All clinical qualifications must be met to ensure compliance. Local candidates are accepted.
