Key Responsibilities

• Assist in the delivery of physical therapy services under the supervision of a licensed Physical Therapist
• Implement established treatment plans and document patient progress accurately and in a timely manner
• Provide hands-on therapeutic interventions as directed by the supervising Physical Therapist
• Communicate effectively with the clinical team regarding patient status and progress
• Maintain compliance with facility policies, credentialing requirements, and documentation standards

Required Qualifications

• Active Physical Therapy Assistant license in Indiana (permanent state license required)
• Prior experience in an outpatient physical therapy setting
• Ability to meet contract credentialing and quality assurance requirements prior to assignment start
• Strong documentation skills and ability to complete required paperwork within established timelines

Preferred Experience / Skills

• Previous experience working in a high-volume outpatient rehabilitation environment
• Familiarity with electronic medical records and clinical documentation systems
