About the Position

We are seeking an experienced Travel Inpatient Pharmacist for a 13-week contract at Providence Hood River Memorial Hospital in Hood River, Oregon. This is a day-shift opportunity for a pharmacist with strong outpatient infusion and oncology experience, responsible for supporting infusion services while collaborating with multidisciplinary healthcare teams to provide safe and effective medication management.

Responsibilities

• Verify and manage orders for hazardous and non-hazardous outpatient infusion medications, including chemotherapy.
• Review and evaluate oncology and non-oncology treatment plans.
• Perform IV infusion medication compounding verification and ensure safe medication preparation.
• Verify insurance authorization before initiating infusion treatments.
• Educate patients on infusion medications, potential side effects, and treatment plans.
• Participate in the management of infusion-related adverse drug reactions.
• Provide medication therapy management for inpatient and outpatient infusion patients.
• Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care.
• Maintain accurate documentation using Epic (preferred).

Requirements

• Active Oregon Pharmacist License (required by start date).
• Minimum 1 year of dedicated outpatient infusion pharmacist experience.
• Oncology patient care experience is required.
• Experience with chemotherapy and hazardous drug handling is highly preferred.
• Strong knowledge of IV infusion medication verification, compounding, and therapeutic monitoring.
• Community hospital experience preferred.
• Epic experience preferred.
