About the Position

Job Title: Physical Therapist Home Health Profession: Physical Therapist Specialty: Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8.5 Experience: 1 year License: Physical Therapist Professional license in Massachusetts Certifications: None specified Must-Have: 1 year Home Care experience Description: Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations, or disabilities resulting from injury, disease, or other causes. Patients may range in age from birth to over 100 years. Collaborate with patient, family, physician, and other allied health professionals to identify patient needs and coordinate services to meet those needs. Utilize thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice. Weekend rotations are required once every 6 weeks. Ability to travel to cases as assigned is essential. Only roles specifically within a certified home health agency will be considered relevant for the 1-year requirement. Experience in hospice, private care, or working as a Case Manager on its own does not meet the qualifications for Certified Home Health Care Management experience.
