Job Title: Home Health Physical Therapist Profession: Therapy/Rehabilitation Specialty: PT Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum of 1 year in home health License: Valid Physical Therapy License required Certifications: BLS, Manual Therapy Must-Have: Proficiency in Oasis Description: Home health physical therapist position available. Candidates must have a minimum of one year of experience in home health. Local candidates are allowed at the same rate. Business casual attire or solid color scrubs permitted. Experience with the OASIS charting system is required. The role involves care for adults and geriatrics. Successful candidates will be expected to manage 5 to 6 patient visits daily. The responsibilities include evaluations, developing and implementing treatment plans, and conducting discharge planning. Team members may be required to float based on facility needs. Candidates should possess strong time management and communication skills. Flexibility in scheduling is essential. Experience with a variety of diagnoses, including but not limited to degenerative joint disease, chronic obstructive pulmonary disease, and neurological disorders is preferred. All applicants should have a solid understanding of therapeutic exercises and functional mobility training. Candidates must be prepared for ongoing assessments and goal setting with patients. Safety and injury prevention practices are paramount in this role. Strong organizational skills and attention to detail are necessary.
