Key Responsibilities

• Deliver physical therapy treatments to residents in a skilled nursing facility setting under the supervision of a licensed Physical Therapist
• Implement individualized treatment plans and document patient progress accurately and in a timely manner
• Assist in the assessment of patient functional abilities and contribute to goal-setting in collaboration with the supervising PT
• Communicate effectively with interdisciplinary care team members to support optimal patient outcomes

Required Qualifications

• Active Physical Therapy Assistant (PTA) license in the state of Virginia or eligibility for licensure
• Graduate of an accredited Physical Therapist Assistant program
• Current CPR/BLS certification
• Experience working in a skilled nursing facility (SNF) setting

Preferred Experience / Skills

• Prior experience in a long-term care or post-acute rehabilitation environment
• Familiarity with electronic medical records (EMR) documentation systems
