About the Position

Job Title: Physical Therapist Profession: Physical Therapist Specialty: Home Health Duration: 13 weeks Shift: 8:00 AM - 4:30 PM Hours per Shift: 8 hours Experience: 2+ years of acute nursing care and home care experience required License: Valid Physical Therapist License Certifications: Active BLS certification from the American Heart Association Must-Have: - Home Health Certification - Proficiency with OASIS - Valid Driver's License - Ability to work independently with minimal supervision Description: The physical therapist is responsible for providing physical therapy services to clients in home health. The primary function includes the provision of direct patient care. Collaboration with other team members is essential in this role. Participation in the development of the comprehensive plan of care for patients is required. Experience with rehabilitation equipment such as splints, orthoses, prostheses, wheelchairs, and ADL equipment is important. Candidates must be home health certified. All requests for time off must be included in the profile prior to submission. Scheduling requests must be made at the time of submission for approval. Candidates are expected to communicate effectively and have voicemail set up for contact purposes.
