About the Position

Job Title: Social Worker Profession: Social Worker Specialty: Social Work Care Coordinator (Discharge Planning) Duration: 13 weeks Shift: 12H Days Hours per Shift: 7a-7p every Wednesday & Thursday and every other Friday, Saturday, Sunday Experience: Minimum 2 years of Acute Inpatient Case Management experience that includes at least 1 year in a large hospital (500+ beds, Level 1) License: Active License - LMSW or LCSW and Active State License Certifications: BLS Certification Must-Have: - Minimum 2 years of Acute Inpatient Case Management experience, including 1 year in a large hospital - Previous travel in a Level 1 facility - Strong computer skills - Epic EMR experience - Acute care/ED discharge planning experience - Recent (within the last two years) acute inpatient case management experience Description: This position requires highly experienced candidates with acute inpatient case management and discharge planning experience. Candidates must be able to pick up quickly and work efficiently, as training on the unit will be minimal. Flu vaccine is required, with only medical exemptions allowed, which requires documented evidence from a healthcare provider. Fingerprinting is required and can be completed remotely or in person. The case load consists of 15-20 patients per day. The interview process will involve a web-based interview. Candidates must comply with specific criteria for complex placements and financial assistance limitations. Team members will work alongside a case management team to ensure that no one is working alone. Charting is done using the Epic system. Weekend and holiday work may be required as needed.
