Job Overview

We are seeking a compassionate and skilled Physical Therapist (PT) to join an interdisciplinary care team focused on long-term, patient-centered rehabilitation services. This role supports older adults and frail populations within a holistic care model designed to maximize independence, mobility, and quality of life.

Unlike traditional high-volume rehab settings, this position emphasizes meaningful patient relationships, collaborative care planning, and sustainable work-life balance.

Key Responsibilities

Conduct comprehensive physical therapy assessments for program participants
Participate in interdisciplinary team (IDT) care planning meetings
Develop and implement individualized rehabilitation and functional maintenance plans
Provide therapeutic interventions including exercise, gait training, and functional mobility support
Deliver treatment modalities such as heat, massage, sound, and electrical stimulation as appropriate
Train and supervise therapy assistants and support staff
Educate patients, caregivers, and staff on therapeutic equipment and mobility strategies
Document treatment plans, progress, and outcomes accurately
Collaborate closely with physicians, nurses, and allied health professionals
Support participant safety and independence goals

Assignment Details

Requirements

Active Physical Therapist (PT) license in California (required)
Bachelor s degree in Physical Therapy (Master s/Doctorate preferred)
CPR and First Aid certification (or ability to obtain within 180 days)
Minimum 2 years clinical experience preferred
Experience with outpatient, inpatient, acute, or skilled nursing populations preferred
Strong communication and interdisciplinary collaboration skills
Ability to work in a team-based, patient-centered care model
