Job Title: Occupational Therapist Profession: Occupational Therapist Specialty: Home Health Duration: Full-time Shift: Monday - Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum one year experience caring for a similar patient population License: Licensed as an Occupational Therapist by the state in which practicing Certifications: Valid Basic Life Support (BLS) Healthcare Provider Card with recertification Must-Have: - Graduate of an occupational therapy education program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) - Eligible to take, or has successfully completed, the entry-level certification examination for Occupational Therapists developed and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT) - Valid licensed driver with automobile insurance in accordance with state and organizational requirements - Mandatory Reporter: Child/Dependent Adult Abuse Description: As an Occupational Therapist in our Home Health team, you will bring your clinical expertise and compassionate care directly to patients in the comfort of their own homes. You will play a key role in developing and delivering personalized therapy plans that help patients regain independence, improve their quality of life, and achieve their goals. Working closely with physicians and the care team, you will provide hands-on treatment, education, and support, all grounded in best clinical practices. Your impact will be seen not only in improved patient outcomes, but also in the meaningful relationships you build and the exceptional experience you create for every individual you serve. Responsibilities include making visits to the homes of patients requiring home health occupational therapy services and assuming responsibility for an ongoing interdisciplinary assessment of the patient. You will evaluate and assess the patient's level of functioning by applying appropriate tests. You will develop specific occupational therapy procedures as well as functional activities in accordance with a written physician plan of treatment and patient evaluation. Additionally, you will develop and administer a plan of treatment including environmental modifications and oversee the use of treatment modalities, prosthetic devices, and therapeutic appliances.
