Contract | Full-Time | Onsite | Monday–Friday, 8:00 AM – 5:00 PM | 13-Week Assignment

Key Responsibilities

• Provide physical therapy assistant services to patients in a home health setting across an assigned territory including Laguna Beach, Aliso Viejo, Dana Point, Laguna Hills, Laguna Woods, and Laguna Niguel
• Deliver patient care in accordance with established treatment plans under the supervision of a licensed Physical Therapistmeets a productivity standard of 30 points per week
• Meet a productivity standard of 30 points per week
• Document all patient care activities accurately and in a timely manner using HCHB (Homecare Homebase) EMR
• Participate in on-call and weekend coverage as required

Required Qualifications

• Active and unrestricted Physical Therapist Assistant (PTA) license in the state of California
• Minimum of 1 year of home health experience with a Medicare-certified facility
• Proficiency with HCHB (Homecare Homebase) EMR system required
• Ability to meet a weekly productivity requirement of 30 points
• Availability for on-call and weekend shifts as needed

Preferred Experience / Skills

• Familiarity with Medicare home health regulations and documentation standards
• Experience managing a multi-city or territory-based patient caseload
