About the Position

Make a meaningful impact as a Licensed Clinical Social Worker in Albany, OR. This opportunity supports hospice and outpatient bereavement services. The role involves advocating for patients and families, coordinating resources, providing grief and loss counseling, and collaborating with interdisciplinary hospice teams to support patients in home settings, skilled nursing facilities, assisted living, and adult foster homes.

Minimum Qualifications

• Bachelor’s degree in social services, counseling, or psychology required. Master’s degree preferred.
• Experience or education in grief or loss counseling required.
• Healthcare Provider Level BLS required.
• Experience working in hospice, home health, or related community settings preferred.
• Current state licensure or credentials as required for the role.

Preferred Qualifications

• Master’s degree in social work (MSW) from an accredited program preferred.
• Previous experience coordinating bereavement programs or hospice social work preferred.

Assignment Details

• Shift hours: 0800-1630.
• Occasional weekends required.
• May include work in patient homes, skilled nursing facilities, assisted living, and other community settings.

Serve patients and families through the final stages of illness by delivering social work services focused on pain and symptom management, medical social services, counseling, spiritual support, hospice aides coordination, and bereavement support. Work within an interdisciplinary hospice team to provide family-centered care across multiple counties and settings.
