Job Summary
We are seeking a licensed Physical Therapist to provide high-quality patient care on a contract basis. The Physical Therapist will evaluate patients, develop individualized treatment plans, provide therapy services, document progress, and collaborate with care teams to support patient recovery, mobility, and overall functional improvement.
Key Responsibilities
- Conduct physical therapy evaluations and assessments
- Create individualized treatment plans based on patient needs and goals
- Provide therapeutic exercises, mobility training, rehabilitation support, and patient education
- Monitor patient progress and adjust care plans as needed
- Maintain accurate and timely clinical documentation
- Communicate with physicians, nurses, caregivers, and other healthcare professionals
- Ensure compliance with clinical, safety, and regulatory standards
- Support patients in improving strength, range of motion, balance, mobility, and independence
- Active Physical Therapist license in Massachusetts
- Degree from an accredited Physical Therapy program
- Current CPR/BLS certification preferred or required
- Prior experience in outpatient, home health, rehab, skilled nursing, or telehealth settings preferred
- Strong documentation and communication skills
- Ability to work independently in a contract environment
