About the Position

Job Title: Physical Therapist - Home Health Profession: Therapy/Rehabilitation Specialty: Physical Therapy Duration: 13 Weeks Shift: Day Hours per Shift: 5x8-Hour (08:00 - 17:00) Experience: Minimum 1 year of Adult Home Health experience License: Must have a valid license in hand Certifications: CPR and BLS Must-Have: - Reliable transportation - Current driver's license - Agency-required liability insurance - Proficiency in HomeCare HomeBase EMR - Proficiency in OASIS within the last 6 months Description: This position involves home visits for therapy services. Periodic office meetings are required. The candidate will be responsible for driving to patients' homes daily. The expected average number of patients per day is 5 to 10, equating to roughly 6 to 8 points. Ideal candidates must be willing to travel up to 75 miles from their home clinic. Mileage will be reimbursed at the IRS standard rate. Expectations require the clinician to accept every patient on their schedule, regardless of distance. Documentation must be completed within 24 hours of the appointment. Delayed or incomplete documentation may result in cancelation. There are no guaranteed hours; the clinician must achieve a weekly productivity of 30 points, equating to full-time hours. All overtime must be pre-approved. Candidates must be open to potential floating to a nearby clinic if patient census is low. Weekend requirements and on-call duties may vary depending on location and profession. Experience in home health is required; no first-time candidates will be accepted. The territory of coverage will be Northern Neck. The work will be primarily home health-focused.
