About the Position

Job Title: Physical Therapist Profession: Therapy/Rehabilitation Specialty: Home Health Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 08:00 - 17:00 Experience: Minimum 1 full year Adult Home Health Experience License: Must have license in hand Certifications: CPR, BLS Must-Have: Recent Home Health experience Description: Therapist will conduct home visits for patients. Periodic office meetings are required. Reliable transportation and a current driver's license are necessary. Agency-required liability insurance is mandatory. Average number of patients per day is 5-6. Contractor will be assigned between 5-10 patients per day. Patients will equate to roughly 6-8 points per day/week. Communication with the account manager is expected for low patient census situations. The contractor must be willing to travel up to 75 miles from the home clinic. Mileage will be reimbursed at the IRS standard rate. The commute to and from the first and last patient is not billable. Candidates must be proficient in OASIS and HomeCare HomeBase EMR. Documentation must be completed within 24 hours of the appointment. There are no guaranteed hours with this contract. All documentation must be timely and complete to avoid cancellations. Candidates must accept each patient on their schedule, regardless of distance. Local candidates are accepted at the same rate. Candidates must confirm comfort with potential travel times between patients. There are no on-call requirements for this position. Previous experience in home health or hospice is essential. Clinicians must not currently be an employee of any applying healthcare facility.
