Key Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility (SNF) setting
• Evaluate, assess, and develop individualized treatment plans for patients
• Deliver therapeutic interventions in alignment with patient goals and care plans
• Maintain accurate and timely clinical documentation
• Float between assigned worksites as needed during scheduled shifts

Required Qualifications

• Active California Physical Therapist license
• Minimum 1 year of experience in a skilled nursing facility (SNF) setting
• Ability to work 8-hour day shifts and float to multiple locations as required
• Ability to meet standard credentialing requirements

Preferred Experience / Skills

• Experience with Casamba documentation software
• Familiarity with multi-site clinical environments
