Job Title: Physical Therapist Profession: Rehabilitation Therapy Specialty: Physical Therapist Duration: 13 weeks Shift: 9-5 Hours per Shift: 36 Experience: Minimum Years of Experience: 0 License: Current state license to practice as a Physical Therapist Certifications: CPR certification preferred or required depending on the facility Must-Have: Graduate of an accredited Physical Therapy program Experience in a Skilled Nursing Facility or geriatric rehab setting preferred Excellent communication, documentation, and interpersonal skills Ability to work independently and as part of a multidisciplinary team Description: We are seeking a compassionate and skilled Physical Therapist to join our interdisciplinary team in a Skilled Nursing Facility. The Physical Therapist is responsible for evaluating residents' physical conditions, developing personalized treatment plans, and delivering rehabilitative care to help patients improve mobility, restore function, and achieve the highest possible level of independence. Key responsibilities include performing thorough assessments of patients' physical abilities, movement limitations, and pain levels. The Physical Therapist will develop and implement individualized treatment plans based on evaluation findings, medical history, and patient goals. Evidence-based physical therapy interventions will be delivered to improve strength, coordination, balance, and mobility. Collaboration with physicians, nursing staff, occupational and speech therapists, and family members will be necessary to coordinate patient care. Monitoring patient progress, adjusting therapy plans as needed, and documenting all care provided in accordance with regulatory guidelines is essential. Education of patients and caregivers on safe mobility techniques, home exercises, and equipment use is required. Ensuring compliance with all regulatory requirements, including documentation, is crucial. Participation in care planning meetings and interdisciplinary team conferences is expected. Supervision of physical therapist assistants and aides as needed is part of the role. A safe and clean work environment must be maintained.
