Social Worker - Acute - (SW - Acute) - Job Description & Requirements Social Worker - Acute - (SW - Acute) StartDate: 02/13/2023Available Shifts: 8 D Pay Rate: $1450.00 - 1900.00 MSW REQUIRED. The Social Worker collaborates with hospital's case management and interdisciplinary healthcare team in an effort to meet facility resource management expectations and enhance patient outcomes. Utilizes professional social work knowledge, relationship skills and interdisciplinary collaboration to assess and intervene in psychosocial issues significant to patient/families and the healthcare team's management of illness, injury and disability. Responsibilities may include but are not limited to: auditing medical records, education of patient and staff, identifying need for psychosocial interventions, assessing discharge planning needs for patients of all ages, identifying opportunities for improvement in patient care, and providing documentation of all functions as required by regulatory agencies. Facility: Eastern New Mexico Medical Center, Roswell, New Mexico Required Qualifications 2+ years of experience. 50-mile radius rule. No more than a 3-month work history gap in the last year.
